PARENTAL CONSENT, INDEMNITY AND LIABILITY RELEASE

FOR PARTICIPANTS YOUNGER THAN 18 YEARS OF AGE
THIS SEPARATE PARENTAL CONSENT, INDEMNITY AND LIABILITY RELEASE MUST BE PRINTED, SIGNED, AND EMAILED TO: INFO@EBASS.CO.ZA

1. Acknowledgement of Risk
   I, the undersigned parent/legal guardian of the minor participant (“the Minor”), acknowledge that participation in any event organised by the Electronic Bass Anglers Sports Society (EBASS) may involve certain risks, including but not limited to personal injury, death, or damage to property. I accept and voluntarily assume, on behalf of the Minor, all such risks.


2. Consent to Participate
   I hereby consent to the Minor’s participation in all EBASS events and related activities and confirm that the Minor is medically fit and capable of participating.


3. Release of Liability
   By signing this agreement, the Participant releases and discharges EBASS, its committee members, Mind Sports South Africa (MSSA) or any other federation or body EBASS is affiliated to, sponsors, agents, officials, employees, and volunteers (collectively “EBASS Parties”), from any and all liability, claims, damages, losses, costs, or expenses of whatsoever nature, arising out of or in connection with the Minor’s involvement in any EBASS event.


4. Covenant Not to Sue
   I undertake, both in my personal capacity and on behalf of the Minor, not to institute any claim, action, or legal proceedings against EBASS Parties in respect of any injury, loss, or damage suffered by the Minor in connection with any EBASS event, save as provided in clause 3 above.


5. Indemnity
   I hereby indemnify and hold harmless the EBASS Parties from and against any and all claims, demands, damages, legal costs, and expenses that may be made against, or incurred by, EBASS Parties arising from:
   • (a) the acts or omissions of the Minor during an EBASS event; or
   • (b) claims made by the Minor, their insurers, heirs, or dependants in respect of injury, loss, or damage sustained in connection with their participation in any EBASS event.


6. Insurance and Subrogation
   I understand that by signing this agreement, I waive, and also waive on behalf of the Minor, any rights that our insurers, heirs, or dependants may otherwise have to claim or institute proceedings against EBASS Parties by way of subrogation or otherwise, in respect of any amounts paid or payable on the Minor’s behalf.


7. Acknowledgement and Understanding
   By signing below, I confirm that I:
   • (a) have read and fully understood this indemnity and liability release;
   • (b) voluntarily agree, both personally and on behalf of the Minor, to be bound by its terms; and
   • (c) have ensured that the Minor has also read and understood the EBASS tournament rules and regulations appropriate to their age and ability.
